UND Honor Band & Choir Festival
Audition Registration Form

Please list the names of your students who will be auditioning for the UND Honor Band & Choir. Students (grades 10-12 only) may audition in both areas ($10.00 per audition). Please provide all requested information for each student. Upon receipt of this list, students will be assigned audition times, and a master audition schedule will be sent to each director.

Directions for multiple entries: Fill out the form completely for the first four students. For any subsequent students, click the browser's back button and fill out only the school name portion of the director information and the complete individual student information.
